Yes, any modern phone with a camera can scan it, iPhone or Android, no extra app needed. Most phones can scan straight from the built-in camera app these days.
Yes, upgrade or downgrade whenever you like from Account > Billing, no waiting for a renewal date. If you downgrade to a plan with a lower QR limit than you're currently using, you get a 14-day grace period to delete or export codes before the extras are archived.
Yes, any time from Account > Billing. You keep full access to your paid plan until the end of your current billing period. After that, your account automatically moves to the free Starter plan instead of being cut off immediately.
